Transfer of Deceased Body to Lebanon نقل جثمان الى لبنان

In order to transfer the body of a deceased person to Lebanon, kindly send the following:

1. Original death certificate;
2. Burial Transit Permit;
3. Letter of non-contagious diseases issued by an authorized health institution;
4. Letter of embalming;
5. Letter of sealed casket;
6. Original passport of the deceased;
7. I.D. (Tazkarat al-hawiya or Ikhraj Kaid) of the deceased;

The death registration form filled and signed by a member of the family and also emailed to seoul.leb@gmail.com with the requested attached documents.

من أجل نقل جثمان متوفى إلى لبنان، يرجى إرسال المستندات التالية:

1. شهادة الوفاة الأصلية.

2. تصريح نقل الجثمان (Burial Transit Permit).

3. شهادة خلو من الأمراض المعدية: صادرة عن مؤسسة صحية معتمدة.

4. شهادة تحنيط الجثمان.

5. شهادة إغلاق وتقرير ختم النعش.

6. جواز السفر الأصلي للمتوفى.

7. الهوية اللبنانية أو إخراج قيد (إفرادي أو عائلي) للمتوفى.

ملاحظة: يجب تعبئة طلب تسجيل الوفاة وتوقيعه من قبل أحد أفراد العائلة، وإرساله عبر البريد الإلكتروني إلى seoul,leb@gmail.com مع كافة المستندات المطلوبة المرفقة.

시신을 레바논으로 운구하기 위해 다음 서류를 제출해 주시기 바랍니다:

1. 사망진단서 원본

2. 시신 인수 및 운구 허가서 (Burial Transit Permit)

3. 비전염성 질병 확인서: 공인된 의료기관에서 발급한 서류

4. 방부 처리 증명서 (Embalming Certificate)

5. 관 봉인 확인서 (Sealing Certificate)

6. 고인의 여권 원본

7. 고인의 레바논 신분증 또는 호적등본 (Ikhraj Kayd)

주의 사항: 사망 등록 신청서를 작성하여 유가족이 서명한 후, 위에 언급된 구비 서류 사본을 첨부하여 이메일(seoul.leb@gmail.com)로 보내주시기 바랍니다.
